Regional Home Visiting Program Models

The following are a few of the locally used evidence based home visiting programs that provide support and education to parents through a trained professional. Click here for a complete list of Home Visiting Evidence based models that are eligible for MIECHV funding

The CHVP funds three evidence based home visiting models – Healthy Families America, Nurse-Family Partnership and Parents as Teachers.

This is a voluntary program supervised by the California Department of Social Services and administered across participating California counties. This program is operated by Ventura Public Health and San Luis Public Health on behalf of the Department of Social Services.

This federally funded evidence based model serves children and families prenatal to age 5, offering once a week home visits

A signature evidence based home visiting model that is a program of Prevent Child Abuse America.

An evidence based model funded in partnership with Health Resource and Service Administration and Administration for Children and Families. The program supports pregnant people and parents with young children who face the greatest risk and barriers to healthy maternal and child outcomes.

An evidence based community health program that has specially educated nurses that regularly visit young mothers starting early in pregnancy and continuing through the child’s second birthday.

An evidence based home visiting model since 1984, Parents as Teachers offers weekly visits and specialized curriculum that supports parent’s role in promoting school readiness and healthy development of children.
